1.  Do not forget to complete the above Sport Insurance Application before making payment using PayPal

2.  There will be a premium increase effective with payments made after 1/01/2010.

3.  If you are uncertain of the effective and expiration date of your “League’s policy”  call us.

4.  Payments for sports teams are 100% earned.  No Cancellations and no refunds.

5. This agency may charge an administrative fee in addition to the commission we receive.  The administrative fee is used to off set the PayPal charge, issuing certificates of insurance and other office expense.  By submitting your application you agree to pay this fee.
